After I ordered GCMS notes, I don't know whether background check and security screening was initiated and completed, but in January 2020 I received request to submit Medicals for my spouse and Kid, who are not accompanying me at this time for family reasons and whose medicals are already submitted and valid till April 2020. At the same time, I received an email from immigration officer asking to confirm whether my spouse and kid are accompanying. I responded to the email and requested for an update on my application regarding time to make a decision. Officer promptly responded confirming medicals no longer needed and said medicals was the one holding up the application. Within few minutes received another email stating application is finalized and wait for instructions on how and where to send passport. A day later, I received the  email! Hopefully this update helps who are in the similar boat.

Since my spouse has a rejection on the change of status from H1b to H4 in US, I've marked as Yes on one of the statuary questions related to my Spouse. As per the GCMS notes, here is what I see:
