Did you do your biometric after MEP?
Did you provide them any explanation in your eAPR- why you did not include the child in your application?
1. Letter/Affidavit from your husband's ex that she's in full custody of the child and that the she continues to retain full custody.
2 A sworn affidavit from your husband that his child will not accompany you now or at any other time in the future to Canada. That he understands the full implications of this on the future immigration of the child to Canada.
It is better to consult an approved Canadian immigration lawyer before appealing. At the same time you can create a new application as well.
Now regarding the Dubai PCC, if you are not currently living in Dubai and you got the PCC after you left Dubai for the last time then you can apply with the same PCC. PCCs issued after you left the country for the last time is acceptable even after the expiry date. The VO will ask for a new one if they need it.

Since all dependents can in future claim a right to come to Canada, they expect everyone to have a medical and hence everyone to be mentioned in the application. As far as I can understand
